The standard protocol involves 100mg–200mg taken at night (as it can cause sleepiness). Women in perimenopause typically use it during the latter half of their cycle (days 14–28); women with unpredictable cycles and those who are post-menopausal use it continuously.
  • Estradiol (Transdermal): Transdermal delivery methods (gels/patches) are considered safer than oral estrogen. While taking estrogen orally can increase the risk of blood clots fourfold, studies (e.g., BMJ 2019) indicate that transdermal application does not carry this elevated risk because the hormone enters the bloodstream directly through the skin. Gels are often used at the beginning for simple dosage adjustments, and it generally requires 4–8 weeks of regular application to notice symptom improvement.
  • Transdermal Testosterone: Testosterone is beneficial for libido and energy levels, but formulations designed for women are not widely available except in Australia. The dosage is minimal (around 300mcg/day), as higher amounts can lead to acne or unwanted hair growth.
  • Dehydroepiandrosterone (DHEA): This is occasionally used as a hormonal precursor, though its effectiveness when taken orally is variable. Note: Vaginal DHEA is intended for treating localized atrophy and not for systemic hormone replacement.
  • Medical Supervision: All forms of HRT must be prescribed and monitored by a qualified medical professional.

The Timeline For Hormone Replacement Therapy (HRT) Effectiveness

  • First Few Weeks You may begin to experience slight relief from symptoms, including better sleep, an improved mood, or more energy. This timing depends on which hormone you begin with. Progesterone tends to work more quickly, whereas Estradiol requires at least 2 to 4 weeks for initial effects to become noticeable. This is also the time to establish your routine for application—when, how, and where.
  • 1 to 3 Months During this period, the dosage is tested, as the therapy is personalized based on your symptoms. If symptoms return, the dose is raised. If you experience breast tenderness or migraines, we may reduce the estrogen. Ideally, your mood swings begin to even out, and your energy levels rise. Symptoms such as hot flashes or night sweats should happen less often.
  • 3 to 6 Months This is the timeframe when most individuals feel substantial relief, as transdermal Estradiol and Testosterone require some time to produce their full effect. Hot flashes, night sweats, and vaginal dryness frequently show dramatic improvement, and mood fluctuations occur much less.
  • 6 to 12 Months You are now experiencing the complete benefits of the treatment and seeing a notable enhancement in your quality of life. You recognize that changes can still occur and are able to adjust the dosage or method of administration as needed.
  • Month 13 Onwards Over the long term, further benefits continue to build, such as enhanced bone strength (lowering the risk of osteoporosis) and improved cardiovascular health, which supports your well-being as you get older. It's important to note that these benefits are most effective when combined with strength training, a nutritious diet, and adequate protein consumption.
