Typically, perimenopausal women use it in the latter half of their cycle (days 14–28), whereas women with erratic cycles and those who are post-menopausal use it on a continuous basis.
  • Estradiol (Transdermal): Topical application methods like gels or patches are considered a safer alternative to oral estrogen. While oral estrogen can increase the risk of blood clots fourfold, studies (such as the BMJ 2019 study) indicate that transdermal application does not carry this elevated risk because the hormone enters the bloodstream directly via the skin. Gels are often prescribed at the beginning to allow for simple dose adjustments, with symptom improvement usually observed after 4–8 weeks of regular application.
  • Transdermal Testosterone: While testosterone can boost libido and energy levels, formulations specifically for women are uncommon outside of Australia. A very low dose is administered (approx. 300mcg/day), since higher amounts can lead to side effects like acne or unwanted hair growth.
  • Dehydroepiandrosterone (DHEA): It is occasionally utilized as a hormonal precursor, though its effectiveness when taken orally can be variable. Note: Vaginal DHEA is intended only for treating localized atrophy and not for systemic hormone replacement.
  • Medical Supervision: A prescription and ongoing supervision from a medical professional are mandatory for all forms of HRT.

What Is The Timeline For Hormone Replacement Therapy (HRT) Results?

The occurrence of symptoms such as hot flashes or night sweats should lessen.
  • 3 to 6 Months Most individuals report substantial relief in this timeframe, as transdermal Estradiol and Testosterone require several months to become fully effective. There is often a dramatic improvement in hot flashes, night sweats, and vaginal dryness, while mood swings occur far less often.
  • 6 to 12 Months You experience the complete benefits of the therapy and see a notable enhancement in your overall quality of life. You also develop an understanding that adjustments may still be needed and can modify the dosage or method of administration accordingly.
  • Month 13 Onwards Over the long term, further advantages accrue, such as increased bone strength (which lowers the risk of osteoporosis) and improved cardiovascular health, contributing to your long-term wellness. It is important to note that these long-term benefits are most effective when combined with strength training, a nutritious diet, and adequate protein consumption.
