Patients undergoing HRT (Hormone Replacement Therapy) often say they “feel like themselves again”. For this reason, determining the ideal treatment rhythm and dosage is a vital investment in the next 30 to 40 years of your life.

A basic guide to help identify which hormone may be low:

  • Low Progesterone: You might experience feelings of anxiety, irritation, and being on edge.
  • Low Estrogen: You could feel dry, overheated, and prone to tears.
  • Low Testosterone: This may manifest as feeling fatigued, physically weak, and lacking drive.

It is important to note that each woman can experience different symptoms with varying levels of intensity. There can also be an overlap or a combined effect between different hormones. The process of identifying and resolving these symptoms is typically a gradual one.

Symptoms Of Hormonal Imbalance Throughout The Body

  • Sleep: Deficient Progesterone leads to disrupted sleep and nighttime awakenings (from a loss of GABA's calming influence); low Estrogen is responsible for night sweats and awakenings in the early morning.
  • Mood: A lack of Progesterone contributes to irritability and anxiety; low Estrogen is associated with depression and anhedonia; insufficient Testosterone diminishes motivation and assertiveness.
  • Menstrual Cycle: Deficient Progesterone results in heavy, erratic bleeding (due to unopposed estrogen); low Estrogen leads to shorter or absent cycles.
  • Vasomotor Symptoms: A deficiency in Estrogen is the principal cause of hot flashes and nocturnal sweats.
  • Libido & Sexual Function: Insufficient Estrogen results in vaginal dryness and pain; low Testosterone is the main reason for decreased spontaneous sexual desire and less intense orgasms.
  • Cognitive Symptoms: Deficient Estrogen is a cause of "brain fog" and memory difficulties; low Testosterone impairs strategic thinking and mental focus.
  • Metabolism: A lack of Estrogen contributes to the accumulation of central body fat (belly fat) and insulin resistance; low Testosterone results in a loss of lean muscle, which in turn slows the metabolic rate.
  • Skin & Hair: Deficient Estrogen leads to a decrease in collagen and skin elasticity; low Testosterone may be a factor in reduced hair thickness.
  • Bone & Muscle: Insufficient Estrogen is a key factor in osteoporosis and sarcopenia; low Testosterone leads to a reduction in physical endurance and muscular strength.
  • Urogenital Health: A lack of Estrogen causes the Genitourinary Syndrome of Menopause (which includes atrophy, itching, and recurrent UTIs).

Understanding The Treatment Process

HRT is an evolving, step-by-step process customized to your individual biology. It differs from a standard prescription, as your physician will modify doses and schedules according to blood work and symptom monitoring. Typically, one hormone is stabilized before another is added, with follow-up appointments eventually scheduled every 3 to 6 months.

  • Progesterone (Oral Micronized): Derived from plants, bioidentical progesterone is readily absorbed. Its advantages include safeguarding the uterine lining, stabilizing hormones, alleviating anxiety, and enhancing sleep quality. The standard dosage is 100mg–200mg at night (to account for its drowsy effect). It is often taken by perimenopausal women in the latter half of their cycle (days 14–28), while women with erratic cycles and those in post-menopause use it continuously.
  • Estradiol (Transdermal): Topical methods (gels/patches) are a safer option than oral estrogen. While oral estrogen can increase the risk of blood clots fourfold, studies (like the BMJ 2019 study) indicate that transdermal application carries no such elevated risk because the hormone is absorbed straight through the skin into the circulation. Gels are often used at the start for simple dose adjustments, and it generally takes 4–8 weeks of regular use to notice symptom relief.
  • Transdermal Testosterone: Testosterone can enhance libido and energy, but formulations made specifically for women are uncommon outside of Australia. The dose is kept very low (around 300mcg/day), as a higher amount can result in acne or hair growth.
  • Dehydroepiandrosterone (DHEA): This is sometimes prescribed as a precursor hormone, though its effectiveness when taken orally can be variable. Note: Vaginal DHEA is formulated specifically for localized atrophy and is not intended for systemic hormone replacement.
  • Medical Supervision: All forms of HRT must be prescribed and monitored by a qualified medical professional.

The Timeline For Hormone Replacement Therapy (HRT) Effectiveness

  • First Few Weeks You may begin to experience slight relief from symptoms, such as better sleep, an improved mood, or increased energy. The exact effect depends on which hormone you start with. Progesterone works more quickly, whereas Estradiol requires at least 2 to 4 weeks for small changes to be noticeable. You will establish a personal routine for when, how, and where to administer the treatment.
  • 1 to 3 Months The dosage is adjusted during this period, as the therapy is personalized based on your symptoms. If symptoms return, your dosage will be increased. If you experience breast tenderness or migraines, we will likely lower the estrogen dose. In an ideal scenario, your mood fluctuations will start to even out, and your energy levels will rise. Symptoms such as hot flashes or night sweats ought to become less frequent.
  • 3 to 6 Months This is the period when most individuals report substantial relief, as transdermal Estradiol and Testosterone require time to become fully effective. Hot flashes, night sweats, and vaginal dryness usually show dramatic improvement, and mood swings happen far less often.
  • 6 to 12 Months You experience the complete benefits of the therapy and see a notable enhancement in your quality of life. You also gain an understanding that changes can still occur and learn to adapt the dosage or method of administration as needed.
  • Month 13 Onwards Further advantages accumulate over the long term, including stronger bones (which lowers the risk of osteoporosis) and better cardiovascular health, contributing to your long-term wellness. Be aware that these benefits are most effective when combined with strength training, a healthy diet, and sufficient protein consumption.
