Anticipation Allows For Prevention

Post-menopause constitutes one-third of a woman's life, which is approximately 33 years, considering the average menopause age of 51 and an average female life expectancy of 84 in the EU. Furthermore, women can experience perimenopausal symptoms for as long as a decade. This means that about half of a woman's life is spent managing various symptoms that can impair her quality of life and productivity.

Individuals undergoing HRT (Hormone Replacement Therapy) often describe the experience as “feeling like themselves again”. Therefore, identifying the correct treatment frequency and dosage represents a crucial investment in one's well-being for the upcoming 30-40 years.

Here is a basic guide for self-assessment to identify potential hormone deficiencies:

  • Low Progesterone: Feelings of anxiety, irritation, and being overstimulated may occur.
  • Low Estrogen: You might experience sensations of dryness, heat, and tearfulness.
  • Low Testosterone: This can lead to feelings of fatigue, weakness, and a lack of motivation.

It is important to remember that symptoms and their severity vary from one woman to another. Hormones can have overlapping or synergistic effects. Addressing and resolving symptoms is frequently a step-by-step, repetitive process.

A Bodily Map Of Hormonal Imbalance Symptoms

  • Sleep: Deficient Progesterone results in disrupted sleep and waking during the night (due to the loss of GABA's calming influence); low Estrogen is responsible for night sweats and waking up early in the morning.
  • Mood: A lack of Progesterone can cause irritability and anxiety; insufficient Estrogen is associated with depression and anhedonia; low Testosterone diminishes motivation and assertiveness.
  • Menstrual Cycle: Inadequate Progesterone leads to heavy, erratic bleeding (from unopposed estrogen); diminished Estrogen causes shorter or missed menstrual cycles.
  • Vasomotor Symptoms: Insufficient Estrogen is the main cause behind hot flashes and night sweats.
  • Libido & Sexual Function: A lack of Estrogen results in vaginal dryness and discomfort; low Testosterone is the main reason for a decrease in spontaneous desire and less intense orgasms.
  • Cognitive Symptoms: Insufficient Estrogen can lead to "brain fog" and memory issues; reduced Testosterone affects strategic thinking and mental focus.
  • Metabolism: Low levels of Estrogen encourage the accumulation of central fat (belly fat) and contribute to insulin resistance; a deficiency in Testosterone results in the loss of lean muscle, which in turn slows the metabolic rate.
  • Skin & Hair: Insufficient Estrogen decreases collagen and skin elasticity; low Testosterone may play a role in thinning hair.
  • Bone & Muscle: A significant factor in osteoporosis and sarcopenia is low Estrogen; reduced Testosterone leads to decreased physical endurance and muscle power.
  • Urogenital Health: Insufficient Estrogen results in the Genitourinary Syndrome of Menopause, which includes atrophy, itching, and recurrent UTIs.

A Look At The Potential Treatment Approach

Hormone Replacement Therapy is an adaptive, step-by-step process customized to your individual biology. It differs from a typical prescription, as your physician will modify dosages and schedules according to blood test results and how your symptoms evolve. The usual approach is to stabilize one hormone at a time before adding the next, with follow-up appointments scheduled every 3–6 months.

  • Progesterone (Oral Micronized): Bioidentical progesterone originates from plants and is readily absorbed by the body. Its advantages include safeguarding the uterine lining, balancing other hormones, alleviating anxiety, and promoting better sleep. The standard protocol involves taking 100mg–200mg at bedtime (as it can cause drowsiness). Perimenopausal women typically use it during the latter half of their cycle (days 14–28), while women with irregular cycles and those who are post-menopausal take it on a continuous basis.
  • Estradiol (Transdermal): Applying estrogen through the skin with gels or patches is a safer alternative to oral forms. While taking estrogen orally can increase the risk of blood clots fourfold, studies (like the BMJ 2019 one) indicate that transdermal application does not carry this elevated risk because the hormone enters the bloodstream directly via the skin. Gels are often used initially to allow for easy dosage adjustments, and it generally takes 4–8 weeks of regular application to notice a reduction in symptoms.
  • Transdermal Testosterone: Testosterone is beneficial for libido and energy levels, but formulations designed for women are not widely available outside of Australia. The required dosage is quite small (approx. 300mcg/day), as higher amounts can lead to side effects like acne or hair growth.
  • Dehydroepiandrosterone (DHEA): This is occasionally utilized as a precursor hormone, though its effectiveness when taken orally can be variable. It's important to know that vaginal DHEA is intended for treating localized atrophy and does not function as a systemic hormone replacement.
  • Medical Supervision: Any form of HRT necessitates a prescription and must be managed under the care of a medical professional.

The Timeline For Hormone Replacement Therapy (HRT) Effectiveness

  • First Few Weeks You may begin to feel slight relief from symptoms, such as an enhancement in your sleep, mood, or energy levels. This depends on the hormone you’re starting with. Progesterone tends to work more quickly, whereas Estradiol requires a minimum of 2 to 4 weeks for minor changes to become noticeable. This period is also for establishing a routine for when, how, and where to apply your treatment.
  • 1 to 3 Months During this time, you and your doctor will fine-tune the dosage, as this therapy is personalized based on your symptoms. If symptoms return, the dosage is typically increased. If you experience side effects like breast tenderness or migraines, we try to reduce the estrogen dose. Ideally, your mood swings will begin to level out and you'll feel more energetic. The frequency of symptoms like hot flashes or night sweats should also diminish.
  • 3 to 6 Months This is the timeframe when most individuals report substantial relief, as transdermal Estradiol and Testosterone require some time to produce their full effects. Issues such as hot flashes, night sweats, and vaginal dryness frequently show significant improvement, while mood swings occur much less often.
  • 6 to 12 Months At this stage, you are experiencing the complete benefits of the therapy and seeing a marked improvement in your quality of life. You have a better understanding that your body is still changing and can make adjustments to the dosage or method of administration as needed.
  • Month 13 Onwards Over the long term, further benefits continue to build, including stronger bones (which lowers the risk of osteoporosis) and improved cardiovascular health, contributing to your overall health as you get older. Keep in mind that these benefits are most effective when combined with strength training, a nutritious diet, and adequate protein intake.

Testimonials

What our customers say about us

Julia, 46 – HRT & Strength
When I saw how much muscle mass I had lost, I finally understood why I felt weaker. It took longer than I expected to get my hormone levels properly adjusted, but once they were stable, my sleep and daily energy improved dramatically. Combining this with regular strength training has made me noticeably stronger and more resilient in everyday life.
Anna, 44 – HRT & Sleep
I never realized how much my hormones were affecting my sleep until I have taken progesterone. After adjusting my therapy and tracking my sleep for six weeks, I went from waking up four or five times a night to sleeping through almost every night. It’s such a relief to finally wake up feeling rested and ready for the day.
Pia, 45 – Menopause & Brain Fog
I thought I was losing my mind. I’d forget simple words in the middle of sentences. Realizing it was just the estrogen drop was a huge relief.
Anna, 49 – Menopause & Sleep
The night sweats were brutal—I was waking up drenched and exhausted every single night. I was so irritable with my family. Once I balanced my hormones, I finally got my sleep back.
Anita, 54 – Menopause & Anxiety
Nobody told me about the sudden heart palpitations and anxiety. I felt on edge for no reason. Learning it was hormonal made so much sense.
