Proactive Prevention Through Foresight

A woman's life post-menopause constitutes one-third of her total lifespan, which is approximately 33 years, considering the average menopause age of 51 and an average EU female life expectancy of 84. Women can also experience symptoms of perimenopause for as long as a decade. This means that about half of a woman's life may be affected by a range of symptoms that diminish both quality of life and productivity.

Individuals undergoing HRT (Hormone Replacement Therapy) often describe the experience as “feeling like themselves again.” Therefore, identifying the correct treatment frequency and dosage represents a significant investment in one's well-being for the upcoming 30-40 years.

Here is a simplified guide to help identify which hormone might be deficient:

  • Low Progesterone: Feelings of anxiety, irritation, and being "wired" can occur.
  • Low Estrogen: You might experience dryness, heat, and tearfulness.
  • Low Testosterone: Symptoms can include feeling fatigued, weak, and lacking motivation.

It is important to remember that each woman's experience with symptoms and their severity is unique. Hormones can have overlapping or synergistic effects. Addressing and resolving symptoms is typically a gradual, iterative process.

Recognizing Hormonal Imbalance: Symptoms by Body System

  • Sleep: Deficient Progesterone leads to disrupted sleep and waking during the night (due to the loss of GABA's calming influence); low Estrogen is associated with night sweats and waking up early.
  • Mood: A lack of Progesterone can cause irritability and anxiety; insufficient Estrogen is connected to depression and anhedonia; low Testosterone diminishes motivation and assertiveness.
  • Menstrual Cycle: Inadequate Progesterone can lead to heavy, erratic bleeding (from unopposed estrogen); low Estrogen can cause menstrual cycles to become shorter or be missed entirely.
  • Vasomotor Symptoms: Insufficient Estrogen is the main cause behind hot flashes and night sweats.
  • Libido & Sexual Function: A lack of Estrogen results in vaginal dryness and discomfort; low Testosterone is the principal reason for diminished spontaneous desire and less intense orgasms.
  • Cognitive Symptoms: Insufficient Estrogen is associated with "brain fog" and memory issues; deficient Testosterone can impair strategic thinking and mental focus.
  • Metabolism: Low levels of Estrogen contribute to the accumulation of central (belly) fat and insulin resistance; a lack of Testosterone leads to a decrease in lean muscle, which slows the metabolic rate.
  • Skin & Hair: Insufficient Estrogen diminishes collagen and skin elasticity; low Testosterone may play a role in thinning hair.
  • Bone & Muscle: A significant factor in osteoporosis and sarcopenia is low Estrogen; deficient Testosterone lessens physical endurance and muscular power.
  • Urogenital Health: Inadequate Estrogen can result in Genitourinary Syndrome of Menopause, which involves atrophy, itching, and recurrent UTIs.

A Glimpse into the Treatment Protocol

Hormone Replacement Therapy is a fluid, evolving process customized to your individual biological makeup. Differing from a typical prescription, your physician will modify dosages and schedules according to your blood test results and symptom monitoring. The usual approach is to stabilize one hormone prior to adding another, with follow-up appointments eventually scheduled every 3–6 months.

  • Progesterone (Oral Micronized): Derived from plants, bioidentical progesterone is readily absorbed. Its advantages involve safeguarding the uterine lining, regulating hormones, alleviating anxiety, and enhancing sleep. The standard protocol involves 100mg–200mg taken at night because it can cause sleepiness. It is commonly taken by perimenopausal women in the latter half of their cycle (days 14–28), while women with irregular periods and those who are post-menopausal typically take it without interruption.
  • Estradiol (Transdermal): Applying estradiol through the skin (using gels/patches) is considered safer than taking it orally. While oral estrogen can quadruple the risk of blood clots, studies (e.g., BMJ 2019) indicate that transdermal application does not carry this elevated risk because the hormone enters the bloodstream directly via the skin. Gels are often used at the beginning for simple dose adjustment, with symptom improvement generally observed after 4–8 weeks of regular application.
  • Transdermal Testosterone: While testosterone can improve libido and energy levels, formulations specifically for women are uncommon outside of Australia. The dose is kept very small (approx. 300mcg/day), since higher amounts can lead to side effects like acne or unwanted hair growth.
  • Dehydroepiandrosterone (DHEA): This is occasionally utilized as a hormonal precursor, though its effectiveness when taken orally can be unpredictable. Note: Vaginal DHEA is intended only for treating localized atrophy and does not function as a systemic hormone replacement.
  • Medical Supervision: Every form of HRT must be prescribed and managed under the care of a medical professional.

Expected Timeline for HRT Results

  • First Few Weeks You may begin to see slight relief from symptoms, like better sleep, mood, or energy levels. This is contingent on which hormone therapy is initiated. Progesterone tends to work more quickly, whereas Estradiol requires a minimum of 2 to 4 weeks before small changes are noticeable. This period is also for establishing your application routine—when, how, and where to apply it.
  • 1 to 3 Months During this phase, your dosage is adjusted, as the therapy is personalized to your symptoms. The dose is raised if symptoms return. If side effects such as breast tenderness or migraines occur, the estrogen dose is typically reduced. The goal is for mood swings to begin to level out and for energy to rise. The frequency of symptoms like hot flashes or night sweats should also lessen.
  • 3 to 6 Months Most individuals report considerable symptom relief during this period, as it takes time for transdermal Estradiol and Testosterone to become fully effective. There is often a dramatic improvement in hot flashes, night sweats, and vaginal dryness, while mood swings occur far less often.
  • 6 to 12 Months By this point, you are experiencing the complete benefits of the treatment and can see a marked improvement in your quality of life. You also recognize that adjustments may still be needed and can modify the dosage or method of administration accordingly.
  • Month 13 Onwards Over the long term, further advantages accrue, including enhanced bone strength (which lowers the risk of osteoporosis) and improved cardiovascular health, contributing to your overall well-being as you get older. It is important to note that these long-term benefits are most effective when combined with strength training, a nutritious diet, and adequate protein consumption.

Testimonials

What our customers say about us

Julia, 46 – HRT & Strength
When I saw how much muscle mass I had lost, I finally understood why I felt weaker. It took longer than I expected to get my hormone levels properly adjusted, but once they were stable, my sleep and daily energy improved dramatically. Combining this with regular strength training has made me noticeably stronger and more resilient in everyday life.
Anna, 44 – HRT & Sleep
I never realized how much my hormones were affecting my sleep until I have taken progesterone. After adjusting my therapy and tracking my sleep for six weeks, I went from waking up four or five times a night to sleeping through almost every night. It’s such a relief to finally wake up feeling rested and ready for the day.
Pia, 45 – Menopause & Brain Fog
I thought I was losing my mind. I’d forget simple words in the middle of sentences. Realizing it was just the estrogen drop was a huge relief.
Anna, 49 – Menopause & Sleep
The night sweats were brutal—I was waking up drenched and exhausted every single night. I was so irritable with my family. Once I balanced my hormones, I finally got my sleep back.
Anita, 54 – Menopause & Anxiety
Nobody told me about the sudden heart palpitations and anxiety. I felt on edge for no reason. Learning it was hormonal made so much sense.
