  • Progesterone (Oral Micronized): Plant-derived and readily absorbed, bioidentical progesterone offers benefits such as protecting the uterine lining, balancing hormones, alleviating anxiety, and enhancing sleep. The typical protocol is 100mg–200mg taken at night (to accommodate its drowsy effect). Women in perimenopause usually take it during the latter part of their cycle (days 14–28), while post-menopausal women or those with irregular cycles use it continuously.
  • Estradiol (Transdermal): Topical methods like gels and patches are safer than oral estrogen. While oral estrogen may increase blood clot risk fourfold, research (like the BMJ 2019 study) indicates that transdermal application does not carry this elevated risk, as the hormone is absorbed directly into the bloodstream via the skin. Gels are often used at the outset for easy dosage adjustments, with symptom relief typically appearing after 4–8 weeks of regular use.
  • Transdermal Testosterone: Testosterone can improve libido and energy levels, though formulations specifically for women are uncommon outside of Australia. The dose is kept very low (around 300mcg/day), as higher amounts may lead to acne or unwanted hair growth.
  • Dehydroepiandrosterone (DHEA): This is sometimes used as a hormonal precursor, but its effectiveness when taken orally can vary. Note: Vaginal DHEA is for treating localized atrophy and is not a systemic replacement therapy.
  • Medical Supervision: All forms of HRT must be prescribed and managed under professional medical supervision.

The HRT Timeline: When to Expect Results

Progesterone generally acts faster, whereas Estradiol requires at least 2 to 4 weeks for initial effects to manifest.   • 1 to 3 Months The dosage is evaluated during this period, as the treatment is customized to your symptoms. If symptoms return, the dosage is adjusted upwards. If you experience breast tenderness or migraines, we may lower the estrogen dose. In an ideal scenario, your mood swings will begin to stabilize and your energy will improve. Symptoms such as hot flashes and night sweats should occur less often.
  • 3 to 6 Months This is the period when most individuals feel significant relief, as transdermal Estradiol and Testosterone need time to become fully effective. Hot flashes, night sweats, and vaginal dryness frequently show dramatic improvement, and mood swings become far less common.
  • 6 to 12 Months You experience the treatment’s full benefits and see a marked improvement in your quality of life. You gain an understanding that changes can still occur and learn to adapt your dosage or administration method accordingly.
  • Month 13 Onwards Over the long term, further benefits accrue, such as stronger bones (which lowers the risk of osteoporosis) and improved cardiovascular health, supporting your well-being as you age. Note that these benefits are maximized when combined with strength training, a healthy diet, and adequate protein intake.
