  • Progesterone (Oral Micronized): Bioidentical progesterone originates from plants and is well-absorbed. Its benefits consist of protecting the uterine lining, balancing hormones, diminishing anxiety, and promoting better sleep. The standard protocol involves 100mg–200mg taken at night (as it can induce drowsiness). Perimenopausal women typically use it in the latter part of their cycle (days 14–28), while women with irregular cycles and those who are post-menopausal use it continuously.
  • Estradiol (Transdermal): Methods that deliver estrogen through the skin (gels/patches) are safer than oral forms. While oral estrogen may increase the risk of blood clots fourfold, studies (like the BMJ 2019 study) demonstrate that transdermal application has no such elevated risk, as the hormone is absorbed directly into the bloodstream through the skin. Gels are often prescribed initially for easier dose titration, and it generally requires 4–8 weeks of regular application to notice symptom relief.
  • Transdermal Testosterone: Testosterone is beneficial for libido and energy, yet formulas specifically for women are uncommon outside of Australia. The dose is kept very low (around 300mcg/day), because an excessive amount can trigger acne or hair growth.
  • Dehydroepiandrosterone (DHEA): It is occasionally prescribed as a precursor hormone, but its oral administration yields inconsistent outcomes. Please note: Vaginal DHEA is formulated for localized atrophy and is not a systemic replacement therapy.
  • Medical Supervision: All forms of HRT must be prescribed and monitored under professional medical guidance.

Timeline for Hormone Replacement Therapy Results

  • First Few Weeks You may start to see slight improvements in symptoms, like better sleep, mood, or energy. The timing depends on which hormone you begin with. Progesterone acts more rapidly, whereas Estradiol needs at least 2 to 4 weeks for initial effects to become apparent. During this time, you will establish your routine for when, how, and where to apply it.
  • 1 to 3 Months The dosage is evaluated during this period, as the treatment is customized based on your symptoms. If symptoms return, the dosage is raised. If you experience breast tenderness or migraines, we may try to lower the estrogen dose. Ideally, your mood swings will begin to level out, and your energy levels will rise. Symptoms like hot flashes and night sweats should become less frequent.
  • 3 to 6 Months This is the timeframe when most individuals feel considerable relief, as transdermal Estradiol and Testosterone need time to take effect. Hot flashes, night sweats, and vaginal dryness frequently improve significantly, and mood swings happen much less often.
  • 6 to 12 Months You are now experiencing the full benefits of the treatment and can see an enhancement in your quality of life. You are aware that changes can still occur and that the dosage or administration methods can be adjusted accordingly.
  • Month 13 Onwards In the long term, more benefits continue to build, such as stronger bones (which lowers the risk for osteoporosis) and improved heart health, which supports your well-being as you age. Note that these outcomes are achieved when combined with strength training, a healthy diet, and sufficient protein intake.
