The standard protocol involves a nightly dose of 100mg–200mg, administered at night because it can cause sleepiness. Women in perimenopause typically use it in the latter part of their cycle (from day 14 to 28), whereas post-menopausal women and those with irregular periods take it on a continuous basis.
  • Estradiol (Transdermal): Applying estradiol through the skin (via gels/patches) is a safer alternative to taking oral estrogen. While oral estrogen can increase the risk of blood clots fourfold, studies (e.g., BMJ 2019) indicate that transdermal application does not carry this elevated risk because the hormone enters the bloodstream directly via the skin. Initially, gels are often prescribed to allow for simple dosage adjustments, and it generally requires 4 to 8 weeks of regular application before symptoms begin to improve.
  • Transdermal Testosterone: Testosterone can improve energy levels and libido; however, formulations specifically for women are not widely available, except in Australia. A very low dose is used (approx. 300mcg/day), since higher amounts may lead to side effects like hair growth or acne.
  • Dehydroepiandrosterone (DHEA): DHEA is occasionally utilized as a hormonal precursor, though its effectiveness when taken orally can be unpredictable. Note: DHEA administered vaginally is intended only for treating localized atrophy and does not function as a systemic hormone replacement.
  • Medical Supervision: Every form of HRT must be prescribed and monitored by a qualified medical professional.

Expected Timeline for HRT Results

  • First Few Weeks You may begin to experience slight symptom improvements, like better sleep, mood, or energy. The timing of this relief is dependent on which hormone you start with. Progesterone acts more rapidly, while Estradiol needs at least 2 to 4 weeks before minor changes are noticeable. You establish your routine for when, how, and where to apply the treatment.

  • Month 13 Onwards Over the long term, further advantages build up, such as improved bone strength (lowering the risk of osteoporosis) and enhanced cardiovascular health, which support your well-being as you get older. It is important to know that these benefits are most effective when combined with a nutritious diet, adequate protein, and strength-based exercise.
