A basic guide to help identify which hormone may be low:

  • Low Progesterone: You might experience feelings of anxiety, irritation, and being overstimulated.
  • Low Estrogen: You could feel dry, overheated, and prone to tears.
  • Low Testosterone: You may notice fatigue, weakness, and a lack of motivation.

Keep in mind that symptoms and their intensities vary from one woman to another. Hormones can also have overlapping or synergistic effects. Addressing and resolving these symptoms is therefore typically a gradual and iterative process.

A Look at the HRT Treatment Process

HRT is a responsive, evolving journey that is customized to your specific biological needs. Unlike a fixed prescription, your physician will fine-tune the dosage and timing based on blood work and how your symptoms are progressing. It's common to stabilize one hormone before commencing another, with follow-up appointments eventually shifting to every 3–6 months.

  • Progesterone (Oral Micronized): Sourced from plants, bioidentical progesterone is absorbed well by the body. Its advantages include safeguarding the uterine lining, balancing hormones, easing anxiety, and enhancing sleep. The standard protocol is 100mg–200mg taken at night (as it can induce drowsiness). Women in perimenopause typically use it during the latter half of their cycle (days 14–28), whereas women with irregular periods and those post-menopause use it continuously.
  • Estradiol (Transdermal): Methods that deliver estrogen through the skin (gels/patches) are safer than oral forms. While taking estrogen orally can increase blood clot risk fourfold, research (such as a BMJ 2019 study) indicates that transdermal application does not have this elevated risk, as the hormone is absorbed directly into the bloodstream via the skin. Gels are frequently used at the start for simple dose adjustments, and it generally takes 4–8 weeks of regular use to notice symptom relief.
  • Transdermal Testosterone: Testosterone is beneficial for libido and energy levels, though formulas created for women are uncommon outside of Australia. The dose is minimal (about 300mcg/day), because an excess amount can trigger acne or hair growth.
  • Dehydroepiandrosterone (DHEA): This is sometimes employed as a precursor hormone, but its effectiveness when taken orally varies. Note: Vaginal DHEA is formulated specifically for local atrophy and is not a systemic replacement therapy.
  • Medical Supervision: Any form of HRT must be prescribed and managed under the care of a medical professional.

Understanding the Timeline for HRT Results

  • First Few Weeks You may begin to experience slight relief from symptoms, including better sleep, mood, or energy. The timing of this varies by hormone; Progesterone tends to work faster, while Estradiol needs a minimum of 2 to 4 weeks for initial effects to manifest. During this time, you establish your application routine—when, how, and where.
  • 1 to 3 Months The dosage is fine-tuned during this period, as the therapy is customized to your specific symptoms. If symptoms return, the dose is raised. If you experience side effects like breast tenderness or migraines, we will attempt to lower the estrogen. Ideally, your mood swings begin to stabilize, and your energy levels improve. The occurrence of symptoms like hot flashes or night sweats should lessen.
  • 3 to 6 Months This is the timeframe when most individuals feel significant improvements, as transdermal Estradiol and Testosterone require time to become fully effective. Hot flashes, night sweats, and vaginal dryness often show substantial improvement, and mood swings happen much less often.
  • 6 to 12 Months You are now receiving the full benefit of the therapy and seeing an enhanced quality of life. You also recognize that changes can still occur and are able to adapt your dose or administration method accordingly.
  • Month 13 Onwards In the long term, more benefits accumulate, such as stronger bones (which lowers the risk of osteoporosis) and improved heart health, contributing to your wellness as you age. It is important to know that these outcomes are best achieved when paired with strength training, a healthy diet, and sufficient protein.
